Liability Waiver and Release Form - 1/ we (the participant and parent/legal guardian) realize that the participation in classes and visiting the farm may result in personal injury. Despite precautions accidents and injuries may occur. By signing this Liability Waiver and Release Form I/we agree to assume all risks and release all liabilities related to or occurring as a result of any and all activities that I/we are involved in as a participant at Elwood Acres of Hope/ Hope Blooms Botanicals. As a participant and/or legal parent/guardian, I agree to release and hold harmless Elwood Acres/Hope Blooms Botanicals, its affiliates, successors, or assigns, directors, parent company, employees, teachers, agents, representatives, from any and all claims, suits, liabilities, judgments, costs and expenses for any personal injury, illness, death, or other loss arising from or related to participation in Elwood Acres/Hope Blooms Botanicals. Furthermore I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to compensating for any damage I/we cause to the facilities utilized by Elwood Acres/Hope Blooms Botanicals. While classes are not drop off, the undersigned gives permission to Elwood Acres/Hope Blooms Botanicals, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. The Liability Waiver and Release form shall be governed in all respects by the laws of the State of South Carolina.
